BACKGROUND: Clear cell carcinoma of the prostate is rare, with the majority of reports consisting of a single case study. CASE: We report a case of prostate cancer with unusual cytoplasmic clearing and a nested-like pattern of growth observed in an 80-year-old patient with lower urinary tract obstruction. Emphasis is placed upon differential diagnosis and the distinction from other clear cell carcinomas arising in the urinary tract, both at a histological and immunohistochemical level. CONCLUSION: Androgen receptor immunohistochemistry seems to be a useful tool to distinguish prostate origin from secondary clear cell tumors.
